In order to display the created Request type, it needs to have a the field summary only as a required field the summary field as well as the attachment field as required fields, and a description field in order to create the tickets in the project.
Solution
You can either create a request type dedicated for this integration, or you can double check required fields on your screen and disable the required confirmation as below
